The Hope Initiative
“Getting Them Home.”
The Hope Initiative provides essential equipment, mobility support, and immediate resources to ensure a safe transition home after hospitalization. Wheelchair accessible transportation for urgent mobility needs during the first 72 hours post discharge.
First Step Forward
“Beginning structured recovery.”
First Step Forward provides 36 outpatient rehabilitation sessions per recipient, including neuro-focused physical therapy, occupational therapy, gait training, and functional mobility work. This tier ensures survivors receive consistent, skilled therapy during the most critical phase of early recovery.
Momentum Scholarship
“Where progress continues.”
Momentum supports survivors who demonstrate commitment and progress by providing 36 additional outpatient rehabilitation sessions. This tier protects early gains, prevents regression, and supports long-term functional improvement.
Transportation Assistance Fund (Reserve)
“Removing barriers to independence.”
Transportation is one of the most significant barriers to consistent therapy attendance. Evolution of Recovery maintains a Transportation Assistance Fund to provide wheelchair accessible transportation on a sliding-scale basis, tailored to distance, frequency, and individual need.
Service availability is dependent on continued funding
